Centura Health AED Grant Application Information and Grant Recipients

As part of our efforts to help place more AEDs into our communities, the Centura Health system is pleased to offer an AED grant funding program entitled Jumpstart a Heart.
Nonprofit organizations encouraged to apply will serve populations where groups of at least 50 gather. Target organizations include elementary, middle and high schools, post-secondary schools, churches, senior centers, camps and community organizations. Grants are awarded to nonprofits for up to $2,500, that are located near the following Centura entities:
Avista Adventist Hospital, Louisville, CO
Littleton Adventist Hospital, Littleton, CO
Parker Adventist Hospital, Parker, CO
Penrose-St. Francis Health Services, Colorado Springs, CO
Porter Adventist Hospital, Denver, CO
St. Anthony Central Hospital, Denver, CO
St. Anthony North Hospital, Westminster, CO
St. Anthony Summit Medical Center, Frisco, CO
St. Mary-Corwin Medical Center, Pueblo, CO
St. Thomas More Hospital, Canon City, CO
The geographical areas include: Denver metro, Colorado Springs, Pueblo, Canon City, Durango, Frisco/Breckenridge area, Winter Park and Granby.
In order to provide direct service to the people these organizations serve, the AED must be housed in a stationary location within a building. The AED must be placed in a prominent and easily assessable location.

Applications will be reviewed by Centura Health, EMS and a marketing representative from one of the geographically corresponding entities listed above. Selected grant award recipients, as well as all applicants, will be notified 45 days following the most recent application deadline. Organizations who qualify but were not selected will be considered at the next application period as well.
Each grant recipient will receive reimbursement for the purchase of one AED, training, first-year maintenance and service fees, up to $2,500. Grant funds will be disbursed to recipients upon proof of purchase via copy of an invoice, purchase order or receipt, as well as a W9. Be sure to include the name of the grant recipient for reference, the name the check should be made payable to, as well as the address reimbursement should be mailed to.

Grant recipients must submit a receipt as well as a W9 in order to be reimbursed for the purchase of an AED. If you have additional questions about the grant program and where to send the invoice and W9, please email aed@centura.org.
